To access the portal you’ll need access to a smartphone or computer that has access to the internet. The process of submitting any documents or applications is best served when uploading them directly to the Self Service Portal. This allows workers to process uploaded documents with ease resulting in a quick turn around – we highly encourage residents applying to utilize this piece of the portal.

Other forms of document submission can be found below, starting with our most preferred methods resulting in the best customer service.

  • Online: via the Self Service Portal. **This is the preferred and quickest method!
  • Mail: 900 East County Road 20 Tiffin OH 44883 – If you need a self-addressed envelope, we will send you one.
  • In-Person: Visit our agency or utilize our drop boxes outside the building.
